Case Management and Care Coordination

Goose Creek Coaching uses a case management approach for all of our services. Each of our clients who receive any of our services is assigned a case manager who provides coordination of services on behalf of the client. Case Management involves the coordination of all services inside and outside the practice to help clients find that right professionals who can help them achieve their goals.

Goose Creek's case managers provide ongoing support in the areas of work, home, school, housing, employment, social relationships and participation in the world around our clients. Our case managers help clients assess their needs, plan their care and implement their goals. Our coaches who serve as case managers provide regular reviews of plans.

Patient Advocacy

Goose Creek Consulting offers the services of patient advocates who act as liasons between patients and their loves ones and healthcare providers with the goal of helping improve or maintain a high quality of care for patients. Patient advocates have experience with legal and healthcare systems and can assist clients with care, planning for their medical care, advance directives for general medication conditions and psychiatric advance directives and in crisis intervention.

Patient advocates help provide information, resources and other information to patients and their loved ones, and help coordinate with medical and allied healthcare providers. Our patient advocates work toward individualized plans for their clients. Our patient advocates have a duty to maintain privacy. Patient advocates enhance the lives and care of our clients, helping patients receive accurate and timely information, the highest standard of care and other benefits.

Educational Advocacy

Goose Creek Coaching provides certified life coaches work as educational advocates in as professionals helping students, families, schools and other relevant parties to assure that each student receives the appropriate education necessary to optimize his or her opportunities to achieve the highest level of potential.  Our certified life coaches also work with some of Northern Virginia's top nueropsychologists who provide diagnostic services and testing recognized by school systems and colleges, psychologists, psychiatrists and others on the student's team. Our approach is individualized to the each student and family.

Our life coaches are familar advocating for students on the elementary, middle school, high school and college levels. In addition, our advocates have experience with the Individualized Education Program (IEP) process, special education services and other issues in Northern Virginia school systems.

Our coaches represent students and families in obtain the best possible modications, accomodations or auxiliary aids that will support the student in participating in and benefiting from all educational programs and activities. At the same time, Goose Creek coaches are able to work with students and families on coping skills that hopefully led to techniques to move forward with fewer accomodations.

Crisis Intervention

Whether you living bipolar disorder or borderline personality disorder, depression or psychosis, Goose Creek Coaching's team leader specializes in crisis intervention. Half of his practice is focused on managing crisis intervention and case management for Goose Creek Coaching. 

Goose Creek Coaching will help you develop a psychiatric advance directive giving doctors requests on how you would prefer to be treated and what modalities you believe are most appropriate in the event that you are psychiatric hospitalized at Dominion Hospital, Inova Fairfax Hospital, Inova Loudoun Adult Psychiatric Services (LAMPS). Mount Vernon Hospital or other facilities in the region. Goose Creek Coaching also has strong contacts at two of the best treatment facilities for substance abuse disorders and dialectic behavioral therapy, Silver Hill Hospital in New Canaan, Ct. and McLean Hospital in Boston. Goose Creek Coaching also helps with admissions to the Renfew Center, for eating disorders, and other programs. Goose Creek Coaching will help with hospital admissions, care coordination between your outside providers and the hospital clinicians, care coordination with your family, aftercare planning with your family and clinicians and action-oriented wall-to-wall followup.

We maintain relationships with the Community Services Boards and other mental health organizations, and are family with the volunteer and involuntary commitment processes. We also visit clients in hospitals and attend commitment hearings.'

Goose Creek Coaching also offers to make referrals to resources, including domestic violence shelters, homeless shelters, psychiatric disabilities services, unemployment services, vocational rehabilitation services, outpatient day treatment programs, residential transitioning living, support groups and other services. We also provide strategies to work with schools and employers. We also provide consultative services to loved ones, families and friends, of those with bipolar disorder, depression, schizoaffective disorder, pervasive developmental disorders and other mental conditions.
